You can get a fillable bank of India deposit Slip/Form from any Bank of India Branch. It has two sides, the left side is for customers and the right side is for the bank itself.
Any customer who wants to deposit money in their bank of India account needs to fill both the sides of the form i.e. left side and right side.

Let's start with customer copy.
Date: You need to write the date of the day when you are depositing money in the bank of India.
Branch Name: You need to write the Bank of India Branch name. You can get this on your Bank Passbook.
Credit Account: Select the correct credit amount from SB/CA/RD/CC/TL. Select the correct account, you will get this information on the bank passbook.
Account Number: You need to fill correct account number. You will get your bank account number in the bank passbook.
Cheque Number: If you want to deposit the cheque amount in your bank account then you need to fill this column, write the cheque number in this column, you will get the cheque number on the bottom of the cheque, otherwise you can leave it. If you deposit cash in your bank account then leave it blank.
Amount: Write the amount of money you are depositing in your bank.
Total: Write the total amount that you deposit in your bank.
Rupees in word: Write the same depositing amount in words.
Now the customer copy section is completed. Now let's fill the bank section.
Write the same information on the bank section. If you are depositing more than 50,000 Rupees then you need to fill in the information of PAN.
If your depositing amount is less than 50,000 then no need to fill in the information of PAN.
Write your full name correctly as shown in Your Bank Passbook.
Do not fill the Transaction ID and Receipt ID as it is filled by Bank, Leave it blank.
Now providing Denominations of notes is very important, so write the denominations in the correct column.
At the bottom right corner provide your signature. One thing that is important here is your signature will match with bank systems.
