4.Write your full permanent address.
5.Write your address where PIN is to be delivered.
6.Write Mobile number, place of birth, Mother's/Maiden Name and Email ID.
7.Write the name to be embossed on debit card, primary account number, secondary account number if any and additional information if required.
Now you have to submit this form in your nearest Andhra bank branch.
